      If we are established CSD students, but attending new schools (not the High School) do we need to submit proof of residence forms? I have children attending the 4/5 and Renfroe for the first time, but I don’t remember being provided with any forms this year. (I have submitted these in the past.)

        There’s a post at the top of the 4/5 website about 4th graders submitting documentation.

        And I think you should have gotten a packet in the mail (we did). If not, try the website as hmmm suggested.
